Double Eyelid Revision Surgery (Revision Blepharoplasty)
Loosened lines, asymmetry, and lowering high "sausage eye" lines

Double eyelid revision surgery (revision blepharoplasty) recreates the double eyelid line when, after a previous double eyelid surgery, the line has loosened, is too high, is not what you wanted, differs between the two sides, or when a scar is noticeable.
The method is decided after checking the previous surgical method, scarring and adhesions, the amount of remaining skin, eyelid thickness and fat, and the line you want.

Quick answer
When and how is double eyelid revision surgery done?
Double eyelid revision surgery is usually done at least 6 months after the previous surgery, once the tissue has softened and settled, though early revision is sometimes done. Revision after incisional surgery is mostly done with the incisional method, but fine line adjustments may be possible with natural adhesion or a partial incision. If a natural adhesion double eyelid has loosened and the eyelids are thin with little skin drooping, revision can be done with natural adhesion again.
What is double eyelid revision surgery (revision blepharoplasty)?
Double eyelid revision surgery recreates the line when, after a previous double eyelid surgery, the line has loosened, is too high, differs between the two sides, or when a scar is noticeable. Adhesions and scars from the previous surgery must be released before the line is recreated, and the amount of remaining skin must also be considered, so accurate diagnosis matters even more than for a first surgery.
When double eyelid revision surgery may be needed
Double eyelid revision surgery is mainly considered when the line has loosened or faded, when the line is so high it looks like "sausage eyes," when you are unhappy with the line and want to change or raise it, when the two lines are asymmetric, or when a scar or multiple creases are noticeable. Before deciding, we first check whether what bothers you is simply swelling that is still resolving.
- Loosened or faded double eyelid line
- High line (sausage eyes)
- Dissatisfaction with the line (changing or raising it)
- Left–right asymmetry
- Scars or multiple creases
If your double eyelids have gotten smaller, what is checked first?
Among patients who come in because their double eyelids look smaller than before, some have a loosened line and others do not. So at consultation we first distinguish between two causes.
| Surgery considered | |
|---|---|
| The double eyelid line has loosened | Double eyelid revision surgery |
| Drooping skin covers the line | Raising the line / removing drooping skin |
Why does a double eyelid line loosen?
A double eyelid line can loosen when the eyelids are thick or you often rub your eyes, and this is relatively more likely with natural adhesion, which does not involve a skin incision. Anatomical research shows that Korean upper eyelids have more subcutaneous fat and fat beneath the orbicularis muscle than Caucasian eyelids, and that this thick fat layer interferes with the structure that forms a double eyelid crease (Jeong et al., 1999). In a 4-year analysis of 1,000 buried suture double eyelid surgeries, 81.87% of revisions were due to suture loosening, that is, loss of the line (Okumura et al., 2025).
If there is ptosis with weak eye opening, the double eyelid line tends to loosen more easily, so in this case ptosis correction is considered along with revision.
Can "sausage eyes" be corrected by lowering the line?
Sausage eyes can be corrected by lowering the line with double eyelid revision surgery, but it is more difficult than a first surgery. The adhesion of the existing line is released and a new line is created at a lower position, and if tissue is lacking, fat or tissue may be added. How far the line can be lowered is limited by the amount of remaining skin and the condition of the scar, and traces of the old line may remain, so accurate diagnosis is important.
| Skin excision method | Two-line method | |
|---|---|---|
| Scar tissue | Removed | Preserved |
| Remaining scar | One line | Two lines (old scar + new incision) |
| Double eyelid fixation | Fixed at the new line | Fixed at the new line |
| Preventing multiple creases (re-adhesion) | Fat repositioning | Fat repositioning |
The skin excision method removes all scar tissue between the old scar and the new double eyelid line, leaving a single scar line. The two-line method preserves the scar tissue while carefully separating the tissue layer by layer, leaving two lines: the old scar and the new incision. In both methods, the double eyelid is fixed at the new line, and fat is repositioned to prevent multiple creases (re-adhesion).
Can double eyelid asymmetry be corrected with revision surgery?
How double eyelid asymmetry is corrected depends on the cause. There are three main causes.
Asymmetry of the double eyelid lines
The height or shape of the two double eyelid lines differs. This is corrected with double eyelid revision surgery to match the lines.
Asymmetry in eye-opening strength
One eye opens more weakly, so eye size and the line look different. This asymmetry is corrected with ptosis correction.
Asymmetry in eye structure
The structure of the eyes themselves differs, such as how prominent or deep-set the eyes are or how thick the skin is. This kind of asymmetry may be difficult to fully match with surgery, so how far to match it is decided through consultation.
When is the right time for double eyelid revision surgery?
Double eyelid revision surgery is usually done at least 6 months after the previous surgery, once the tissue has softened and settled. However, early revision is sometimes done. If it has not been long since surgery, we first check whether your current appearance is due to swelling that is still resolving. If there is infection or the line has changed suddenly, it is best to be seen first regardless of timing.
What should I check when choosing a clinic for double eyelid revision surgery?
Revision must take into account scarring and adhesions from the previous surgery and the amount of remaining skin, so accurate diagnosis matters even more than for a first surgery. The points to check at consultation are summarized in our guide to choosing an eye surgery clinic.
How does the revision method differ by the previous surgery?
If a natural adhesion double eyelid has loosened
If a natural adhesion double eyelid has loosened and the eyelids are thin with little skin drooping, revision can be done with natural adhesion again. If it loosened because the eyelids are thick or have a lot of fat, or if it has loosened several times, it may loosen again for the same reason, so the incisional method is often recommended.
If the previous surgery was incisional
Revision after incisional surgery is mostly done with the incisional method, because adhesions and scars from the previous surgery must be released and the line recreated. However, fine line adjustments may be possible with natural adhesion or a partial incision.
What is checked before double eyelid revision surgery?
Before revision, we check these seven points to decide on the method.
- The previous surgical method (natural adhesion or incisional)
- The location and extent of scarring and adhesions
- The amount of remaining skin
- Eyelid thickness and fat, and how many times the line has loosened
- The line you want
- Eye-opening strength
- Left–right asymmetry
Even with the same loosening, the method differs if these seven points differ. If eye opening is also weak, ptosis correction may be planned at the same time.
Double eyelid revision swelling and recovery time
Swelling after double eyelid revision surgery often lasts longer than after the previous surgery, but not in every case. When a high "sausage" double eyelid is lowered to a natural line, recovery can sometimes be faster than after the previous surgery. Aftercare is the same as for the first surgery.
2–3 days after surgery
- Swelling and bruising are at their peak
First 3–5 days
- Sleep with your head elevated
- Cold compresses for the first 7 days
After 3 days
- Makeup is allowed on areas without wounds
Around 1 week
- Incisional: stitches removed, face washing from the next day
After 4 weeks
- Makeup on the sutured area and contact lenses are allowed
- Alcohol and smoking can be resumed, no earlier than 4 weeks after surgery
Double eyelid revision side effects and limitations
What are the possible side effects of double eyelid revision surgery?
After double eyelid revision surgery, temporary swelling, bruising and a foreign-body sensation are common; rarely, left–right asymmetry, loosening of the line, a line higher or thicker than planned, scarring, infection or hematoma can occur. With revision, how much can be corrected is limited by the amount of remaining skin and the condition of the scar, and traces of the old line may remain.
Double Eyelid Revision Surgery (Revision Blepharoplasty) FAQ
If a natural adhesion double eyelid loosens, can it be redone with natural adhesion?
If a natural adhesion double eyelid has loosened and the eyelids are thin with little skin drooping, revision can be done with natural adhesion again. However, if it loosened because the eyelids are thick or have a lot of fat, or if it has loosened several times, it may loosen again for the same reason, so the incisional method is often recommended. The method is decided after checking the existing double eyelid and the condition of the remaining tissue.
After incisional double eyelid surgery, can revision only be done with an incision?
Revision after incisional double eyelid surgery is mostly done with the incisional method. Adhesions and scars from the previous surgery must be released and the line recreated, which is often difficult to correct with natural adhesion. However, if the line has only faded slightly or needs a fine adjustment, natural adhesion may be possible, and this is decided at consultation after checking the existing line and scar.
Can an incisional double eyelid be undone or removed?
An incisional double eyelid can be released or softened with revision surgery, but it is difficult to fully return to the original monolid. The adhesion that forms the line is released, and fat or tissue is placed in between so it does not reattach. The incision scar will remain, and a faint crease may also remain. Rather than removing the line entirely, changing it to a lower, natural line is also considered.
Does recovery from double eyelid revision surgery take longer than the first surgery?
Swelling after double eyelid revision surgery often lasts longer than after the first surgery. It depends on scarring and tissue condition from the previous surgery, and also differs by method. Aftercare such as face washing, makeup, and avoiding alcohol and smoking is the same as for the first surgery.
How is the cost of double eyelid revision surgery determined?
The cost of double eyelid revision surgery depends on the previous surgical method, the problem to be corrected (loosening, a high line, asymmetry, scarring), whether fat or tissue is added, and whether combined procedures such as ptosis correction are done. The extent of revision surgery varies widely with your eyelid condition, so the exact cost can be given after consultation.
